Insurance coverage for treatment by doctors/surgeons out-of-state?

These expenses are referred to as out of network. They will be considered for payment but at a different rate than in network costs. Out of network costs you more. The criteria for the expenses is the same. If your plan prohibits cosmetic surgery, but covers an appendectomy, the out of network costs for an appendectomy will be paid but the costs for cosmetic surgery will not.
